Thank you for your interest in submitting a nomination for the
Chancellor’s Student Service and Leadership Awards program. Please read about
the award categories before proceeding.
Chancellor’s Student Service Award (CSSA) – recognizes students
who have made a significant contribution to the university, region, nation, or
world through community service. Student nominees must have contributed a
substantial amount of time, effort, and creativity, and have a minimum GPA of
Eugertha Bates Memorial Award – given in recognition of the
student(s) whose volunteer activities, whether on campus and/or in a community
setting, demonstrate an outstanding level of caring, dedication, and selfless
commitment to others. Nominees must have exhibited an exceptional contribution
to a humanitarian need.
President's Volunteer Service Award - given to the student(s)
whose service substantially addresses a need in the community. The student must
be involved in the creation of a new project or service initiative to address
particular need(s). The awardee(s) should demonstrate the highest level of
dedication and selfless commitment to their volunteer service and should have
accumulated a minimum of 100 hours of service in any involvement in the past
Activities Honorary Society (AHS) Induction - The University of Illinois Alumni Association (UIAA)
recognizes undergraduate students whose participation in a variety of student organizations
on campus has contributed to a better UIC community. AHS nominations will be reviewed separately from CSSLA
nominations, by past AHS inductees, and with the guidance of the UIAA. A special new inductee ceremony will occur
during the Chancellor's Student Service and Leadership Award Program.
All students selected to be honored at the Chancellor’s Student Service and
Leadership Awards program will receive the Chancellor’s Student Service Award
(CSSA) recognition. The Eugertha Bates Memorial Award and
the Presidential Volunteer Service Award are given to the few most deserving
students as chosen by a selection committee.
Minimum requirements for all awards:
• Currently enrolled student at UIC
• Overall minimum GPA of 2.5 as indicated in the student’s university academic record

Page 4 (optional) will allow you to nominate students to be considered
for induction into the Activities Honorary Society, one of UIC's
longest-standing honors and traditions.
Students will be automatically prompted via email to
respond with their contact information to ensure accuracy and to complete their
nomination. Note: They must provide this information in order for their nomination to be complete and considered for an award.
Complete the following information for the student you are nominating (or
about you for a self-nomination). A UIN and the student's preferred email are
required to notify the student that he or she has been nominated and to
collect/confirm required contact information from the student
to complete this nomination.
